Correction

An article published on 15 January 2018 headlined 'Press the button if you are still alive' alleged that a service failure led to the death of a resident in England.

We are happy to clarify that the OkEachDay system worked successfully and the system did alert the council in question but the alert was not acted upon by the council due to an internal failure which resulted in the resident not being visited.
