Hospitals failing to record patient details

NHS Lothian is one of the worst health boards in Scotland for keeping tabs on patients' ethnicity.

A report by ISD Scotland found that health chiefs were only noting ethnic background in about seven per cent of cases.

Experts say it is a key way of discovering groups which are not receiving equal care and identifying certain health problems.
