Hospitals warned on oxygen procedures
HOSPITALS were urged yesterday to check oxygen equipment after errors, such as wrong dosages and faulty equipment, were found to have contributed to at least nine deaths since 2004.
The National Patient Safety Agency issued guidelines England and Wales in the light of 281 incidents.
