When I read Dr Richard Smith’s grimly ironic comment about “death by cancer” I knew that he would be berated by those ­deficient in the fabled British sense of humour.

In fact, when I became a parish cleric almost half a century ago my parishioners were afraid they might get cancer and die – by the time I left 40 years later they were afraid they might not get cancer but live on to get Alzheimer’s.
