Long-term vision for NHS
A RANGE of measures have been introduced to improve the care of people with long-term health problems in the Lothians.
The strategy will modify the care framework of people with illnesses like lung and breathing difficulties.
For many patients, the Edinburgh Community Health Partnership is now able to reduce the number of unnecessary emergency admissions to hospital and provide a single point of contact for patients and carers.
Ruth Burns, of NHS Lothian, said: "Edinburgh CHP has developed a three-year action plan."
