Errors with X-rays led to delays in lung cancer diagnosis

THE diagnosis of two patients with lung cancer was delayed after mistakes in checking X-rays, a report found yesterday.

Overall, a "critical incident review" of X-rays and scans performed at Lorn and Islands Hospital in Oban found 47 cases where it was possible that significant findings were missed.

The review started after concerns were raised about the work of consultant radiologist Dr James Murray when he worked at hospitals in Ireland.

Concerns were also raised by a GP practice about the accuracy of X-ray reports from Dr Murray.

External experts reviewed X-rays and ultrasound scans carried out by two radiologists, including Dr Murray, between October 2007 and May 2008. They found two cases where diagnosis of lung cancer was delayed, but did not believe this affected the patients' care or outcome. In a further 43 cases with discrepancies, they "did not impact on patient management, care or outcome", the report said.

In addition the review found that in two ultrasound scans, patients' problems were not spotted – one who had gallstones and the other kidney stones. This meant they suffered months of "prolonged discomfort", but it did not affect their recovery, the report said.

Dr Ian Bashford, NHS Highland's medical director, said: "The review re-assessed all chest X-rays, barium examinations, all X-rays performed on children aged under 12 years and all ultrasound investigations reported by consultant radiologists at the hospital from 16 October 2007 to 15 May 2008."

He added: "The overall incidence of potential missed pathology falls within the limits of professional acceptability previously laid down by the Royal College of Radiologists and other professional bodies."
